KODAK Lens Derby Dash for Kash 2019

Start 2019 with a chance to earn some extra cash for your practice. Register to run the Derby Dash for Kash from Kodak Lens and PracticePlus®. Your practice could then earn up to $25 for each incremental Kodak Progressive Lens pair redeemed between January 1, 2019, and May 31, 2019.

How Your Practice Could Earn Cash Back 

• Your practice must be a PracticePlus member. Not a member? Visit Practice-Plus.net, or call (800) 950-5367 to join.
• Download the Derby Dash Registration Form
• Select your Practice’s Race Group
  • New Single-Location Practice
  • Current Single-Location Practice
  • New or Current Multi-Location Practice
• Return the Completed Registration Form by January 31, 2019.
• Redeem Kodak Progressive Lenses between January 1, 2019 – May 31, 2019

First, second, and third prize will be awarded to practices with the most incremental Kodak Progressive Lens pairs redeemed.  Last year’s Derby Dash winners earned over $52,000 in total rewards.  Here’s what you could earn this year on each incremental pair.*

*Total rewards include the $10 per pair of Kodak Progressive Lenses you will receive through PracticePlus. Incremental pair means actual pairs of Kodak Progressive Lenses sold during the contest minus your baseline number of pairs. Your baseline is the number of Kodak Progressive Lenses redeemed 1/1/2018 – 12/31/2018, divided by twelve (12) and multiplied by five (5). All PracticePlus requirements for participation and minimum redemptions shall apply. Deadline for contest entry – 1/31/2019.
