• Download the Summer Splash for Kash Registration Form
• Select your Practice Group:
  • Group 1: New Single-Location Practice
  • Group 2: Current Single-Location Practice
  • Group 3: New or Current Multi-Location Practice
• Return the Completed Registration Form by July 31, 2019.
• Redeem Kodak Progressive Lenses between July 1, 2019 – November 30, 2019

*Must enroll in PracticePlus. Total rewards include the $10 per pair of Kodak Progressive Lenses you will receive through PracticePlus. Incremental pair means actual pairs of Kodak Progressive Lenses sold during the contest minus your baseline number of pairs. Your baseline is the number of Kodak Progressive Lenses redemption activity from  7/1/2018 through  11/30/2018. All PracticePlus requirements for participation and minimum redemptions shall apply.
